Our office hours are Monday - Friday from 8:30 am to 5:00 pm. We can see your child by appointment only.

If your child is ill and you wish to have him seen, it is best to call early in the morning so that we may accommodate your scheduling needs. Please advise the office staff regarding each child needing an appointment.

If your child has a true emergency (such as severe trauma or a convulsion), call 911.

Please telephone between 8:30 am and 5:00 pm Monday through Friday to make appointments or to ask brief questions. The phones tend to be busiest in the early morning, especially on Mondays and Fridays. You may wish to wait until the late morning or early afternoon to call for issues that do not require immediate attention. When calling the office please follow the directions given by our voice mail system, as this will help us handle your call more efficiently.

One of our nurses will be able to answer many of your questions. If you prepare your information and questions before calling, we will be able to handle your call in the most efficient manner. If physician consultation is needed, your child will be given an appointment with one of the doctors.

Your appointment is reserved exclusively for you. If you are unable to keep the appointment, please give 24 business hours notice in order that another patient may be given that time. We reserve the right to charge for appointments that are not cancelled in a timely manner.
We ask that you make sure to get all necessary prescriptions written at the time of your office visit.

If you are on daily medications for a condition such as ADHD/ADD, asthma or allergies, we will be prescribing the amount necessary until your next follow-up appointment. (Generally 3 to 6 months, depending on your child’s diagnosis).

Many of our patients have insurance changes every year. If you need new prescriptions because of a change in your insurance, you will need to schedule a follow-up visit with the doctor.

Every insurance plan has its own formulary. If you want to make sure to have a prescription prescribed with the lowest pharmacy co-pay, you need to bring in your formulary and discuss this with the doctor at the time of each visit.

If there is a medical necessity for a prescription to be called in after-hours, a $15.00 fee will be charged. This applies to all prescriptions called in after hours and on weekends, prescriptions called in without an office visit, and prescriptions called in or written due to formulary changes separate from an office visit. This fee will not be billed to your insurance company, and will be your responsibility.

The direct lines to our office close at 5:00 pm. After 5:00 pm weekdays and on weekends, you may call 766-3344 and the voice mail system will relay messages to the physician on call. Please reserve these calls for sudden or serious problems that cannot wait until the following business day. This time is not for routine questions that can wait until regular office hours. For the fastest service and the best medical care, please follow these guidelines:

Be sure to leave the phone number where you can be reached.
Please keep the line open for the doctor to return your call.
If you do not receive a return call within 90 minutes, please call again.
Have the name and phone number of your pharmacy ready.
Think about your child's illness for a minute before calling so that you can quickly and accurately describe the symptoms to the doctor.
On Saturday mornings, the physician on call may see your child if the illness is too severe to wait until Monday. Please call the office before 9:00 am if you feel that your child needs to be seen in the office. An after-hours charge will be assessed for appointments made after regular business hours and on weekends and holidays. This charge may not be covered by your insurance company. Please remember that while we provide round-the-clock coverage for our patients, we expect that calls made after hours will be for urgent problems. Parents should be able to give temporary treatment to non-emergency conditions (colds, fevers, cough, stomach upsets, etc.).

For your convenience, our Patient Health Questionnaire is now available online. You may print it and fill it out prior to your initial visit.

It is our office policy to perform annual physical examinations on all children beginning with their second birthday. If you have a form to be completed, please bring this with you to your child's examination. Please complete as much of the form as possible, including the immunization history, before you come to the appointment.

We will complete your child's school, sports and camp forms if a physical exam has been performed within one year and we will date it accordingly. When the form requests information, please complete as much of the form as possible, and send it to our office with a self-addressed stamped envelope. We will verify the information according to the last physical examination and forward the document to you. Please allow 2 weeks for completion of these forms.

Our practice is a part of Central Ohio Primary Care, Inc., and all billing is handled by our central office.

We participate in several managed care, HMO and PPO insurance plans. If you are covered by one of these plans, we require payment of your co-pay or coinsurance at the time of your visit. We will then bill your insurance company for the balance due. If you are covered on a traditional plan or a plan in which we do not participate, you will be expected to pay for your services at the conclusion of your visit unless previous arrangements have been made. As a courtesy we can submit a claim to your insurance company once these services have been paid. You will receive a billing statement for any amount due from you after your insurance company considers your claim. Prompt payment helps control expensive billing costs, a savings that we can pass along to you. If necessary, special financial arrangements can be made with our office manager. Extended payment plans need to be discussed in advance.

Please remember that your insurance coverage is an agreement between you and the insurance company. We will bill according to the information provided to us and we will routinely ask you for a copy of your current insurance card. If there is a dispute in this billing, please contact the insurance company before calling our central billing office.

Your insurance company may request access to any relevant medical records (or copies of those records) as necessary to process a claim. This may include cases handled by self-insured organizations, insurance company audits, hospital associations or peer review programs.

The physicians may also release copies of your child's records to a specialist or other health care provider to whom your child has been referred. This will aid in the continuity of care.

We understand that there are times when a parent is unable to bring their child in for a necessary appointment. In order for our office to provide services in this situation, it is necessary that written consent accompanies the individual who will be presenting with the child. The consent requires a parent's signature and a daytime phone number of the parent(s). This consent will be kept in the patient's chart.
