IMPORTANT:
Before calling to schedule
an appointment with us, you MUST pre-register
EACH child on the Ohio Department
of Health website to obtain
each child's unique ID number.
Please bring your registration
papers with you at the time of your
appointment. We cannot make any exceptions
in order to be in compliance with
the Ohio Department of Health.
Call
between the hours of 8:30 AM - 5 PM
(M-F) for appointments.
Appointments
are made on a first-come first-serve
basis. Please do not call after hours
to schedule appointments for the H1N1
vaccine.
